MyVytalCare – An Overview

MyVytalCare is a patient-owned personal care record that is accessible to the patient via either their phone or a personal computer. MyVytalCare focuses on where the NHS is hurting, where the majority of healthcare spend is being made – Long-Term Conditions – where 30% of the population account for 70% of the healthcare spend - £70 billion per annum.

MyVytalCare provides a range of solutions in improving patient care including automated management of identifying patients with due and/or overdue health checks and then automated call and recall according to NICE recommendations. The result - significantly reduced interventions such as unnecessary hospital admissions and saving the NHS £ billions of Pounds per annum.

In addition to the Call and Recall features, MyVytalCare offers many other benefits:

  • It is multilingual – a patient simply selects a language and the text automatically changes;

Automated Call and Recall

The majority of the 17 million patients with Long-term Conditions have not had their recommended health checks resulting in unnecessary admissions to hospital. MyVytalCare’s Call and Recall function is fully automated, ensuring improved patient compliance with NICE recommended health checks. Patients requiring health checks or other actions such as immunisations or MRads, whether due or overdue, are automatically identified and alerted and then seamlessly communicated with via the MyVytalCare App on their phone or PC. The result - GP Practices saving considerable time and resources, and reduced admissions to hospitals and other services. The bottom line is significant cost saving to the NHS and time saving and increases in QOF scores to GP Practices.

Additional MyVytalCare Features

  • Booking Online Appointments: MyVytalCare will enable patients, on being prompted to book an appointment for their health check, to book online appointments with their practice.
  • Authorised Users Can Add Data to the Patients Personal Care Record: MyVytalCare enables authorised users such as consultants, carers and family to add data to the patients record. This information will be sent back to the GP system.
  • Directory of Services:  We will publish your local directory of services in MyVytalCare ensuring that on recommending a patient to have a health check, MyVytalCare will signpost patients to the correct service, saving unnecessary phone calls to the CCGs or practices for information. In addition, time and cost savings will be achieved by directing the patient to alternative services to the GP practice.
  • Credible Knowledge Summaries:  MyVytalCare provides patients with automated up-to-date clinical guidance for common conditions and symptoms, empowering them to self-manage conditions like Diabetes, Hypertension and Asthma, whilst avoiding unnecessary visits to the GP. 
  • Links to Wearables and Remote Monitoring Devices:  MyVytalCare will interface with wearables and remote monitoring devices, which will automatically link to and drive personal care records and personal care pathways.  This will increase patient motivation and provide integrated prevention and wellbeing in single record.
  • Security, Confidentiality and NHS Governance Standards Guaranteed:  Through MyVytalCare, personal care records are securely hosted on the NHS N3 Network under the same exacting standards as GP and hospital records.  All MyVytalCare data transmissions are encrypted.

Reporting and Analytics, Performance Monitoring

MyVytalCare provides a real-time record of patients’ conformance with LTC NICE (or local) care pathways, by LTC, by practice, by comorbidity, by patient group, by age, gender and location. This results in real time identification of patients not conformant with LTC NICE (or local) care pathways, allowing for more focussed and timely interventions to improve conformance. Leads to reductions in non-elective admissions, including patients from care homes. Higher conformance leads to lower costs and better outcomes.
